7. What documents do you need to submit for your Doctoral Degree?

Once you successfully defend your dissertation, you and your department must complete the Ph.D. Form II: Report on Final Examination for the Ph.D. Degree. Make sure all doctoral members have signed the form, as well as your Department Chair or Graduate Advisor. Also, the Associate Dean's signature is required for students in the Schools of Biological Sciences, Engineering, Humanities, Social Ecology, and Social Sciences.

Dissertation/Thesis Approval Form
As the title suggests, the Dissertation/Thesis Approval form is used by University Archives to indicate that they have accepted your manuscript.

Graduate Student Diploma and Commencement Form
The information you enter on the Diploma and Commencement form is used for your entry in the commencement program and for completion of your diploma.

Optional Forms (OGS)

Degree Certification Request Form
The Degree Certification Request form is used to request an official letter from UCI's Graduate Dean certifying that you have completed all of your degree requirements.

For more information about how to complete your dissertation (including formatting requirements), please visit the University Archives website, and "Where can you find information/help to write your thesis/dissertation?" on this website.

IMPORTANT:When you are ready to submit your dissertation, University Archives will provide you with the URL for the online UCI Doctoral Exit Survey (REQUIRED), and a Survey of Earned Doctorates form (REQUIRED).
